Preparticipation
Physical Evaluation for Athletes
The following forms must be turned in to the Athletic Office before a student is allowed to participate in a Dade Christian Varsity or JV athletic team. Please ensure that the Medical History is completed BEFORE visiting your doctor, and that all signatures are provided.
